| Description |
Administrative forms are documents which may be generated by the database for
printing, but are not associated with specific data. They may actually need to be created
prior to the beginning of student record entry, and to aid in that process. There are four
such forms available: |

| Gold Sheet |
This is a document used by the classroom teachers to nominate students to be tested
for Art Visions. |
| Yellow Sheet |
This is a parent permission form which must be signed in order for students to
be tested. |
| Nomination Letter |
The art teacher sends this form to parents of students who have been nominated
by the faculty for testing. It is attached to the yellow sheet. |
| Nomination Guidelines |
This is form is alternatively attached to the pink sheet for use by parents who
wish to nominate their children for testing. |
| Testing Instructions |
A single sheet listing detailed testing instructions for grades arranged in four categories:
2, 3, 4-5 and 6-7. The contents of Task Two, a drawing exercise, may be controlled by any
of 4 pulldowns in the yearly data file. |
| Interview Schedule Sheet |
A simple slotted list of 15-minute time blocks for student and auditions interviews to be
printed as needed and used in when necessary. |
| Poster |
An ad-sized poster on letter paper advertising Art Visions nominations and declaring a due
date. |
